1989 Palabora South Africa Fatal pulley drum defect was determined by use of FEA prior to fabrication.
1988 Kennecott – Bingham Canyon United States After startup, the drum of a tensioner pulley failed in the housing. CDI was not involved in the design of the pulley. CDI investigated the failure of the shell and determined that the shell is sub-designed. An engineering audit of the remaining pulleys was performed, using the CDI failure criteria, and similar deficiencies were found in the high tension pulleys. These pulleys failed three months after our audit.
1986 Quintette Hydraulics Canada The overall system was experiencing controlled braking runaways, gear reducer and pulley failures, and numerous other problems. CDI was contracted to identify, analyze, and correct the problems.
